What is Medicare?
Medicaid is the largest source of health coverage in the United States, jointly funded by the federal government and the states. As of March 2026, it covered roughly 74 million people nationwide.1 Each state runs its own program within federal rules, which is why eligibility and exact benefits can look a little different depending on where you live.
For most enrollees, there's no monthly premium, and co-pays are low or nonexistent. Medicaid pays providers directly, so covered care generally shouldn't show up as a bill in your mailbox.

Primary care, specialists, and checkups. Most visits are free.
Prescriptions
Insulin, blood pressure medication, and mental health prescriptions are covered.

How Medicaid works
Hospital care
Emergency visits, surgeries, and hospital stays, without the giant bill.
Eligibility is generally determined by household income and size, along with factors like age, disability status, and whether you're pregnant or caring for children. Each state sets its own income limits and decides which optional benefits to offer on top of the federal minimum, which is why two neighboring states can have different rules.
You can apply directly through your state's Medicaid agency or through the Health Insurance Marketplace.3 Once you apply, the state reviews your information and lets you know whether you've been approved. If you have, coverage can start immediately and, in many states, can even apply to medical bills from the few months before your approval came through.
